Health History Form & Immunization Record

• Helps us provide safe and appropriate healthcare.

• Must be completed and electronically signed by a parent or guardian for campers under 18.

• Immunization Policy:

  • All staff and campers must follow Wisconsin school vaccine requirements.

  • Exemptions may be accepted for medical, religious, or personal convictions.

  • During outbreaks of vaccine-preventable illnesses, Camp Jorn YMCA may deny specific vaccine exemptions.

• Special Health Considerations:

  • If your child has conditions requiring special care—such as anaphylaxis, severe asthma, seizures, Type 1 diabetes, gastrointestinal disorders, motor disorders, ADHD, autism, cognitive disorders, or other exceptional health conditions—please contact our CEO, Katy Bost at Katy@campjornymca.org as soon as possible to ensure we can safely meet your camper’s needs.

Other Required Forms

• Authorized Grown-Ups Form – Lists who can pick up your camper; campers are not released to underage or unverified adults.

• Day Travel Form – Choose arrival/departure method (car or bus). Bus spots limited.

• Camper Photo – Helps staff recognize campers and assist in emergencies.

• Bullying Contract-

Campers cannot attend without all completed forms.

At Camp Jorn, we believe every child should have the opportunity to experience camp. Scholarships and fundraising options are available to help families of all sizes and income levels.

Scholarships

• Typical Awards: Families may qualify for up to 30% off the base rate.

• How to Apply:

  1. Complete the online camper application.

  2. Submit a Scholarship Application with a $50 refundable deposit per camper.

     • Charged to the default card on file after submission.

     • Applied toward camper balance; refundable if canceled at least 2 weeks before the session.'

• Scholarships are made possible by the generosity of donors, alumni, community members, and volunteers.

Fundraising Opportunities

Camp Jorn provides optional fundraising opportunities for families to help offset camper costs:

• Partner programs with Meadow Farms, Flower Power, and other vendors allow families to earn funds toward their camper balance.

• Participation is optional, and all proceeds go directly toward your child’s camp experience

Payment Options:

• Deposit: $30 non-refundable per session, applied to balance. Balance due on June 5th at noon.

• Pay in Full: Full balance at registration.

• Pay As You Go: Make payments at your convenience. Any remaining balance will be automatically charged to the card on file after 12:00 PM on June 5th. Please note that campers cannot attend until the balance is paid in full.

• Installment Plan: Automatic charges on 1/29, 2/27, 3/27, 4/30; final payment 6/5.

• Custom Payment Plan: Parents/Guardian's may request a custom payment plan for campers attending four or more weeks of Day Camp. Please note: If your camper’s last day of camp occurs before the payment plan end date, the final payment will be due the Friday prior to their last scheduled week. Please note your payments will be charged to the default card on file as follows: 6/12, 6/26, 7/10, 7/24; Final payment on 8/7.

Discounts:

• Multi Week Discount: If your camper is attending four or more full weeks of camp, you’ll receive a $15 discount per session!

Refunds:

• The balance (excluding the deposit) is refundable if canceled at least 2 weeks prior to your camper’s session start date. Exceptions may be made for verified medical reasons.
• Campers are not eligible for refunds or credits for partial or single day absences and or illnesses.

AVW Shuttle Service

Bus Service: Available from Arbor Vitae–Woodruff Elementary (AV-W) for Day Camp. Select weeks during registration under Additional Options.

Fee: $50 per child/week
Supervision: Camp Jorn staff accompany campers on the bus.

Location: Back parking lot at AV-W, 11065 Old 51 N, Woodruff, WI 54568

Schedule:

• Morning: Load 7:10 AM | Depart 7:30 AM

• Afternoon: Depart Camp Jorn 4:30 PM | Arrive AV-W 5:00 PM

Check-In:

• Park in designated spot behind the bus/along sidewalk.

• Campers stay in vehicles until staff check them in, in order of arrival.

Check-Out:

• Remain in your vehicle; staff will verify ID and dismiss campers in arrival order.

Notes:

• Pick-up by 5:30 PM.

• Changes (emergencies, weather) will be communicated via email and Facebook.

• If not picked up by 5:30 PM and no contact is made, campers return to Camp Jorn for pick-up.

Participation Requirements

To help ensure a safe and positive experience for all campers, children enrolled in our program must be able to:

• Stay with their group at all times.
  For everyone’s safety, campers must remain with their assigned group. Staff must maintain proper supervision ratios and cannot leave other children to locate a camper who has left the group.
• Participate in structured activities and transitions independently.
  Our program operates at a ratio of 1 staff member to 12–15 campers. We are not able to provide one-on-one or small group support.
• Use the restroom independently.
  Campers must be fully toilet trained and able to use the bathroom without assistance. Staff are not permitted to help in the restroom or change soiled clothing.
• Engage in outdoor activities for extended periods.
  Most of our programming takes place outdoors, including in light or moderate rain. In the case of thunder or lightning, we will move indoors until conditions improve. Campers should be comfortable spending time outside, encountering insects, hiking, and playing in a variety of weather conditions.

If a camper is unable to meet the expectations listed above, they may be removed from the program. Please note that families remain responsible for the weekly fee in these cases.

• Offers t-shirts, sweatshirts, snacks, water bottles, and more.

• Recommended deposit:  Honestly this amount is up to you! Your camper will have the opportunity to visit our camp store through out their time here. We ask that you let your camper know each week how much money you placed into their account and how to make it last.

• Refunds issued for balances of $10+ in the fall

• Campers should not carry cash

Add funds via Camp InTouch account → View Camp Store → Fund Account If you have more than one camper and enter the same amount in each account within a few minutes you may get a "declined" transaction because your credit card company thinks it is a duplicate transaction.  Try entering a different amount for example, if you entered $20 on Julia's enter $20.50 in on Sara's!

Pro Parent Tips

• Please apply sunscreen and bug spray before your child comes to camp.
• Don’t be alarmed if you get a call from camp. We like to get parent input on even minor health, homesickness, and behavioral issues.
• Dress your child for a day of outdoor fun. Please do not send your child’s best clothing/shoes, as camp activities are outside and rugged. All items should be clearly labeled with your child’s full name. This will assist us with claiming lost and found
• If your child is especially prone to mosquito bites, be sure to send long layers (long sleeve shirts/ pants) to help prevent future discomfort.
